2010 Disney Tip Forms

  • Medication Authorization Form (PDF)
    You need to complete the first page, but may want to read the second page as well. If prescription medication is being sent on the trip, the signature of a health care provider must be on this form.

  • Medical Chaperone Letter (Word | PDF)
    Please read and contact Mr. Lloyd with any questions. There will be a medical chaperone on each of our 4 buses who will be responsible for dispensing medication. Therefore, it will not be possible to choose a specific person to dispense your child's meds. However, you will be made aware of your child's primary chaperone, the chaperones on their bus, and the specific medication chaperone on the bus on the evening of the spring trip. If you have specific concerns or directions about dispensing your child's medication, you can speak to their medication chaperone following the trip meeting.

  • Medication Chaperone Agreement (Word | PDF)
    This requires a parent signature along with the signature of the medication chaperone, director and principal. You will need to sign this form and turn it in, after which it will be signed by the medication chaperone for your child.
